OWL Award Nomination Form

At Samaritan, our long term care employees provide compassionate care while connecting with residents on a daily basis. The OWL (Outstanding Workers in Long Term Care) Award is a way to honor those caregivers who have contributed an extraordinary level of dedication, and shown exceptional service to Samaritan while consistently demonstrating the HEART values, which are Honesty, Empathy, Accountability, Respectful, and Trustworthy.

Nominations are being accepted. If you know of a caregiver who you believe to be an exceptional candidate for the OWL award, please complete the form below. A committee who will determine the awardees throughout the year will view nomination forms. Patients, staff, volunteers, visitors and community members are welcome and encouraged to nominate a caregiver they feel is deserving of this honor.
