- Copy of Immunization records
- TB test* for experience requests longer than 24 hours
- Flu shot record
- Proof of a physical in the last year.

Next, please review the following educational requirements. You will be responsible for all information cover therein. At the conclusion of your review of the materials, please fill out the educational attestation and send it to [email protected]. For those under 18, please also have your guardian sign the parental consent form and email it to [email protected] as well.
